Appointment Procedure

The office door will be locked. We ask that you call the office when you arrive and remain in your car. We will call you when it’s time for you to enter the office. For the protection of everyone, we respectfully ask that except for parents, intravenous sedation escorts and other necessary care givers, other companions don’t accompany you.
For the protection of everyone, patients, intravenous sedation escorts and parents/legal guardians must wear a face mask when entering the reception area. . Due to the limited supply of face masks, we ask that you use your own cloth face mask.
Patients, parents/legal guardians and IV sedation escorts will have their temperature taken. Patients will be asked to complete and sign a short questionnaire about Covid-19 symptoms. Depending upon temperatures and patient answers, the patient’s appointment may need to be rescheduled.
The intravenous sedation escort MUST enter the reception room with the patient and must stay in the reception Room throughout the patients surgery and recovery. 

Restrictions: Patients and parent/legal guardians must wear a face mask when entering the Exam room for examination/consultation, post-operative care or follow-up. Unless the patient is a minor, only the patient will be allowed in the Exam room. Only the patient will be allowed in the Surgery room for treatment/surgery and must wear a mask when entering the Surgery room.
We will be wearing masks and gloves throughout your office visit and using different types of Personal
Protective Equipment (PPE) including face shields, depending upon the nature of your visit or treatment to be performed.
We do have one serious and personal request. If you are diagnosed or think you may have come in contact with COVID-19 within 14 days of your visit with us, please call and let us know. It could be possible that despite feeling fine when you were here, you could have already had the virus.

In order to minimize your time in the office and risk of exposure, please print, complete and bring the completed forms with you to your appointment. Failure to do so may result in a need to reschedule your appointment.
